The current criteria limit orthodontic treatment to enrolled child members age 20 or younger who qualify through prior authorization for a severe handicapping malocclusion.

Who may qualify under standard Health First Colorado
HCPF says braces may be available for a child with a severe bite problem. DentaQuest's current criteria are more specific: the member must be age 20 or younger, actively enrolled, and approved through prior authorization for a severe handicapping malocclusion. Crowding or a desire to improve appearance by itself is not enough.
The standard adult dental summary lists orthodontia as not covered for members age 21 and older. An adult should not start treatment expecting payment based on the children's EPSDT benefit or a general statement that Colorado Medicaid includes dental care.
The orthodontist builds the prior-authorization record
Only a provider properly enrolled with an orthodontic specialty designation can be reimbursed for Health First Colorado orthodontic treatment. The current criteria require the provider to submit the applicable Colorado index form and diagnostic materials such as radiographs, models or their digital equivalent, and appropriate photographs.
DentaQuest makes the determination after reviewing medical necessity and EPSDT requirements. The criteria discourage requests for mild or moderate malocclusions unless a concurrent medical condition warrants review under EPSDT. Families should describe functional and medical concerns accurately, but should not try to self-score the index.
- Ask whether the initial evaluating provider is enrolled for Health First Colorado orthodontics.
- Ask what records will be taken and whether the evaluation itself is covered.
- Ask when the complete request was submitted and how the written decision will be delivered.
- Do not allow banding or active treatment to begin on the assumption that approval is pending.
Approval has dates, eligibility, and continuity rules
An authorization is effective only for its written span dates. The current DentaQuest criteria say an approved orthodontic prior authorization is valid for 1,080 days, but it becomes unusable if treatment begins after the approved span or the member is no longer eligible for the Health First Child plan. The determination letter controls the individual case.
If treatment is delayed beyond the authorization dates, the provider must submit a new request before starting. If the member changes orthodontists, continuation-of-care paperwork and a new authorization for the receiving enrolled orthodontist may be required. Never assume an approval transfers automatically between practices or states.
CHP+ and Cover All Coloradans use different rules
CHP+ is a separate program. Its current member page says braces are limited to treatment necessary to correct a bite or jaw problem and lists 12 continuous months of CHP+ coverage before orthodontic treatment. CHP+ uses its own criteria and authorization process; the July 2026 removal of its former orthodontic dollar maximum did not remove those requirements.
DentaQuest's current Health First Colorado member page says the Cover All Coloradans dental benefit for children ages 0–18 does not cover orthodontics. Do not infer orthodontic coverage from a child's age until the exact program shown in PEAK and DentaQuest is confirmed.

Identify the exact program and renewal date
Confirm active standard Health First Colorado, CHP+, or Cover All Coloradans coverage and the member's age on planned service dates.

Start with an enrolled dentist or orthodontist
Use the DentaQuest directory, then call to verify the specialty, location, plan, new-patient status, and referral process.

Wait for the written determination
Ask for the approved or denied services, span dates, and any conditions. An office estimate is not the authorization.

Use appeal rights if needed
Read the denial letter and act by its deadline. Ask DentaQuest what reconsideration, internal appeal, or state-hearing route applies; additional clinical documentation may be relevant but does not guarantee reversal.
