Apply for Health First Colorado dental coverage

Dental offices do not decide Medicaid eligibility. Apply through Colorado's official system, provide accurate information, and track the application through PEAK or the state contact center.

Four official ways to apply

  • Online: begin at the Health First Colorado Apply Now page, which routes to Colorado PEAK.
  • Phone: call 1-800-221-3943 or State Relay 711 during the hours shown on the official page.
  • Mail: download the current paper application from the official page and follow its delivery instructions.
  • In person: use a county office or an official application-assistance site.

Prepare accurate household information

The application asks for information needed to evaluate household members under current eligibility rules. Use the state's “What do you need to apply?” checklist because required information can vary by person and program.

Cover All Coloradans allows qualifying children and pregnant people to apply regardless of immigration status. Use HCPF's program and privacy materials instead of assuming a household member cannot apply.

After you submit

    Keep the application or case number and a copy of documents you submitted.

    Respond through the official channel if the state asks for more information by a deadline.

    The official Apply Now page says applicants can monitor status online.

    Read the eligibility notice

    The notice states who was approved, the coverage dates, or what review rights are available after a denial.

Once coverage is active

  • Create or update your member accounts and keep contact information current.
  • Review the DentaQuest dental materials for the member's age and program.
  • Use the official dentist directory, then call the office to verify new-patient availability.
  • Give the office the member information so it can check eligibility for the date of service.
  • Do not assume care received before the effective date is covered; ask the state or DentaQuest about the exact dates and any retroactive eligibility shown in the case.
