Colorado Medicaid dental forms and official downloads

Use this curated library to reach the program document—not a copied form. Always confirm the revision date and whether a notice applies to the member's exact program.

A valid official form can still be wrong for a different member group or date of service. Match the document to Health First Colorado, CAC, CHP+, or another program.

01

Current dental benefit summaries and notices

The adult and child summaries are the best quick references for covered categories, common frequencies, authorization language, exclusions, and member contacts. For adults, use the July 2026 revision that shows the $3,000 July–June limit. For standard children, use the current child summary showing no annual dollar maximum and EPSDT language.

02

Member portals, directories, and handbooks

Use the member portal for individual tools such as benefit information and Dental Home details. Use the dentist finder to identify possible offices, then call the exact office because directory data cannot guarantee network status, service, age range, or appointments. The handbook explains broader rights, responsibilities, appeals, contacts, and program processes.

03

Cost disclosure, appeals, and decision help

A participating dental provider uses the non-covered-service disclosure before a member agrees to pay for truly non-covered care. It is not a blanket waiver for covered services. For a denial or other adverse decision, use the appeal directions and deadline in the member's own notice, supported by current DentaQuest and HCPF guidance.

04

Language, disability, and accessible-format resources

Health First Colorado offers free language assistance and auxiliary aids. Ask for an interpreter or accessible document through the official program contact, and ask the dental office what it can arrange for the appointment. Do not rely on a machine translation for a consent, denial, or appeal deadline when official language help is available.

05

Download and share forms safely

  • Open the document from its official HCPF, Health First Colorado, DentaQuest, CMS, or HHS domain.
  • Check the revision or effective date and compare it with the planned date of service.
  • Confirm the program and member group before using a rule from the document.
  • Save an unedited original and a copy of anything completed or signed.
  • Send protected information only through a channel named by the plan, agency, or provider.
  • If a link moves or a current official source conflicts, use the update center and report the broken or conflicting statement through the correction policy.
